New Student Registration
New Student Registration Requirements
To register a new student in the Montgomery Independent School District, a Parent or Legal Guardian must be present at the time of registration. A current driver’s license and/or picture identification must be available to copy for student file.
Other requirements:
- birth certificate
- Social Security Card
- last report card (or withdrawal form)
- proof of residency
- Need any two of the following: utility bill, lease agreement, contract to purchase home (including closing date), or real estate tax certificate. Please note that one company with two more more utility services is only considered one proof of residency.
- IF parents/guardians are living with another family within our district there are additional forms required, which includes a notarized statement with “proof of residency” requirements attached.
- immunization records
In the case of legal custody matters regarding divorced parents, these documents should be made available for student records.
